ISN Support Services Brokerage is a leader in the transformation of health and human services for adults with intellectual and developmental disabilities (I/DD) and believes in self-determination, the ability for people to determine and direct their own lives. ISN staff work collaboratively with customers to identify and access resources to address their support needs and person-centered goals.
ISN’s core values of Love, Justice, Truth and Freedom along with the Service Commitments, adapted from the organizational model for trauma-informed care known as Sanctuary, form the foundation of the ISN service philosophy.
ISN serves individuals in eight of Oregon’s counties: Marion, Benton, Clatsop, Columbia, Lincoln, Polk, Tillamook and Yamhill.

Our History:
Child Youth and Family Integrated Services Network was founded by four established social service entities based in the Willamette Valley in the Mid-1990’s. Together, Catholic Community Services of the Mid-Willamette Valley, Chehalem Youth and Family Services, Rainbow Family Services and Polk County Mental Health created a platform to encourage care coordination in social services.
Integrated Services Network opened the Support Services Brokerage in 2001 with 126 customers in 8 Oregon Counties.
In 2019, Catholic Community Service assumed sole ownership of the brokerage. In 2024, we obtained nonprofit status and currently serve 557 individuals across the eight counties.

Catholic Community Services is a non-profit, faith-based organization. Founded in Salem in 1938, CCS offers 12 programs across ten Oregon counties and serves more than 5,000 people each year. Dedicated to providing the highest quality services, trauma-informed care and research-based practices, CCS is nationally accredited by the Council on Accreditation for Child and Family Services, Behavioral Health Services and Group Living Services.
