The Provider Process

Step 2

Submit Required Documents

We will need a valid copy of your identification to order a Criminal History Check. If you plan on providing transportation services we will need copies of a valid driver's license and auto insurance.

Step 3

Complete the Online Criminal History Check (CHC)

You will receive an email from ORCHARDS with instructions for completing the CHC.

Step 4

Complete the DHS Provider Enrollment Application Agreement (PEAA)

Return the completed application to the Oregon Department of Human Services

Step 5

Complete Carewell Training

You will need to complete an in-person Carewell Orientation Training to receive your provider number. Once received, you will be able to complete the two additional training courses that are required.

Step 6

Complete Public Partnerships (PPL Packet)

PPL is the fiscal agent the State has contracted with to process payroll for PSWs and take care of Employer related tax requirements.

Step 7

Complete the Personal Support Worker Information and Acknowledgement Form and Mandatory Abuse Reporting training with an ISN staff member

Sign a Service Agreement

You will participate in a meeting with the Individual served, Common Law Employer, and the ISN Personal Agent to review and sign a Service Agreement. No paid services may be provided to the Individual until the Service Agreement is signed by all parties.
