Serious Incidents

Serious Incidents must be reported to the individual’s PA within one business day of the incident. The events can be reported via phone, email or face to face.

Per OAR 411‑317‑0000 (190), an event is a Serious Incident if any of the following occur:

1. Physical Aggression: The person intentionally tries to harm someone, and someone is injured (including the individual themselves).

2. Death: Any death under any circumstances.

3. Emergency Medical Care: The person receives emergency medical care for serious illness, serious injury, psychiatric emergencies, or domestic/sexual violence.

4. Emergency Physical Restraint: Use of restraint that is not part of a Positive Behavior Support Plan or a safeguarding intervention.

5. Missing Person: The individual is missing longer than the time limit in their ISP.

6. Psychiatric Hospital Admission: Any admission to a psychiatric hospital.

7. Injury from Safeguarding Intervention/Equipment: If a monitoring device, safety device, or safeguarding action causes an injury.

8. Suicide Attempt: Any attempt, regardless of medical outcome.

9. Unplanned Hospitalization: The person is hospitalized unexpectedly.

10. Medication Error with Adverse Consequences: A medication mistake that harms the person or requires emergency treatment or a call to the prescriber.
